Long Term Care
New Franklin Center’s long term care unit is an especially warm and welcoming place. Here, we care for residents who can no longer live independently and are in need of daily medical care.
Our goal is to help our residents maintain their quality of life by spending time in a nurturing environment where all their every day needs are looked after and they can form warm relationships with staff and fellow residents. Resident’s enjoy meals, activities, exercise and spiritual peace in a place they are happy to call home, with the security of residential assistance and skilled nursing.
Our long term care staff provide 24-hour nursing and custodial care for senior and chronically ill residents. We also offer rehabilitative, restorative, and ongoing skilled nursing care to residents who need assistance with daily tasks.
Our approach to long-term care is focused on delivering exceptional quality of care and enhancing the quality of our resident’s life. New Franklin’s Medical Team includes a physician and nurse practitioner who are at the facility daily. The Clinical Team is comprised of licensed nurses, certified nursing assistants and therapists.
Upon admission, we engage the resident and family caregiver to understand their goals, values, and wishes. Recognizing and appreciating the goals determined by the resident and their family, the staff collaborate to create a Personalized Care Plan appropriate for each individual resident.
